Strength Training in Pregnancy May Lower Gestational Diabetes Risk

Expectant mothers who engage in structured strength training during pregnancy may experience a reduced risk of developing gestational diabetes alongside a significant boost in their overall quality of life, according to a new clinical study. The research highlights the potential benefits of resistance exercise for pregnant women, particularly those who are overweight or living with obesity.

The physical training initiative commenced between the 12th and 14th weeks of pregnancy, running through to the 36th week or until delivery. The program combined weekly supervised exercise sessions with home-based workouts, encouraging the participants to train three times weekly using resistance bands or light weights.

The trial revealed that only 2% of the women in the strength training cohort developed gestational diabetes, compared to 7.3% of those in the control group. While this overall difference did not reach statistical significance, the results were even more pronounced among the highly compliant participants. Within the exploratory subgroup of women who adhered closely to the program and exercised at least twice a week, none developed gestational diabetes. However, researchers cautioned that this was a small subgroup and does not definitively prove a preventive effect.

Beyond diabetes risk, the exercise program delivered clear benefits for maternal well-being. Participants in the strength training group reported a significantly higher quality of life and suffered fewer common pregnancy complaints, such as back or pelvic pain, anxiety, and fatigue. Crucially, the physical exertion did not elevate the risk of complications, as rates of miscarriage and preterm birth remained comparable between both groups.

“Our findings suggest that adherence to a structured strength training program during pregnancy may help reduce the risk of gestational diabetes while also improving maternal well-being,” noted Teresa E. Santa Cruz, MD, the corresponding author of the study from Hospital Universitario Donostia in San Sebastián, Spain. Dr. Santa Cruz added that additional research is necessary to validate these findings and establish the most effective ways to integrate strength training into standard prenatal care.

In a randomised clinical trial published in Acta Obstetricia et Gynecologica Scandinavica, researchers studied 209 pregnant women who were overweight or had obesity. The women were randomly assigned to either a strength training program or usual prenatal care.

She said further research was needed to confirm the findings and determine “how strength training can best be incorporated into routine prenatal care.”
